The Four Star level of Achieve WELL requires that all of the Three Star criteria be satisfied: A 4 Star level requires:
- At least one program is implemented to address a high risk or disease management issue,
- Five “wellness” activities for employees,
- Evidence of community involvement by your organization,
- Documentation of 1 new safety program/or increased emphasis of key safety programs to address issues from needs assessment, and
- Proof of at least 24 months of continuous program activity.
- At least one employee will have begun coursework towards completion of a Wellness Council of Indiana approved Wellness Training Certification.
Key personnel will be administered a behavioral assessment in order to fully understand each individual and teams behavioral styles and preparedness for change.
